Obtaining Your Medical Records
MyChart Patient Portal
The easiest and fastest way to access your medical information is using MyChart, SRHS’s convenient and secure patient portal. Your MyChart account provides a wide range of tools and information that help you access and manage your records. MyChart is password protected and delivered via an encrypted connection, so all information is safe from unauthorized access.

If you can’t find the information or medical record documents you need in MyChart, you can request your medical records from our medical records services team by calling us or submitting an authorization form.

Step 1: Complete the “Authorization to Release and Disclose Patient Information” form by downloading it below.
NOTE: Signature on form must be done with a pen. Virtual/digital signatures will not be accepted.

Step 2: Submit the form, along with a photocopy image of a valid state- or federal-issued photo ID, one of four ways:
